Hannam writes:Following a period of time as an in-patient in the LRI clients may be discharged in the City to the Younger Disabled Unit (YDU) at the Leicester General Hospital or a Nursing Home as part of the care pathway leading to discharge into the community.
Julie Page is currently very involved at the in-patient stage but Headway always felt that once patients were discharged to the above units our involvement decreased unless, where possible, a place at Headway House was commissioned by Health.
Under new funding arrangements we will now be in a position to begin to address this situation as it will now be possible to offer in-put within these establishments for some patients just embarking on rehabilitation. We shall be liaising closely with the Brain Injury
Team to assess potential clients in the first instance and I shall then begin programmes of activities on a weekly basis. Initially this will involve 1:1 negotiation regarding need and interests but where possible I would like to create some sessions involving more than one client that could lead to ongoing interaction throughout the week and not be reliant on my weekly session.
I hope to be able to offer a range of interesting activities and will aim to set up a specific room/ private area for this to take place so clients feel they are ‘moving on’ into a new and different situation with a specific purpose rather than being ‘visited’ once a week. I will also aim to develop good working relationships with YDU and nursing home staff and where appropriate offer training/suggestions for on-going activity throughout the week.
This is an exciting new initiative which will enhance our vision of a ‘seamless’ Headway service and I welcome the opportunity to return to direct contact with members.
